This is the official plugin made by Code Rubik to embed a questionnaire made with one of our applications below:

Participants fill in the questionnaire you created directly from your website and you can view their answers from your app account.

Couldn't figure out how to use it, no instructions

After installing, I couldn't find anywhere in WordPress to actually make the poll. I checked the PlugIn site and got a 404 error and there weren't any instructions on the WordPress plug-in page. I just wound up deactivating it. The developer needs to fix that link and let people know how to use it before it can be of any use.
